Kemik Yoğunluklu İnhomojen Fantomda Küçük Alan Ölçümleri ile Doz Hesaplama Algoritmalarının Vertifikasyonu

Özet

Radyoterapide doz dağılımını hesaplamak için Monte Carlo (MC), Collapse Cone (CC),
Pencil Beam (PB) algoritmaları kullanılır. Algoritmaların doğruluğu ve küçük alan ölçümleri,
inhomojen ortamlarda doz dağılımının doğru hesaplanması için kritik öneme sahiptir. Bu
çalışmada doz hesaplama algoritmalarının inhomojen ortamlarda dozimetrik farklılıklarını
araştırmak için, CC04 (IBA, Almanya) iyon odası ve EBT-XD (Ashland, ABD) film ile inhomojen
fantomda ölçümler alındı ve tedavi planlama sistemi (TPS) ile karşılaştırıldı. CC04 iyon odası ve
EBT-XD film ile çekilmiş olan bilgisayarlı tomografi (BT) görüntüleri TPS’e aktarılarak 1x1, 2x2,
3x3, 4x4 ve 5x5 cm2 alanlarına ve 6MV-FFF, 10MV-FFF enerjilerine göre hesaplamalar yapıldı.
Daha sonra aynı fantom düzeneği Elekta Versa HD lineer hızlandırıcısında kurularak ölçümler
alındı. MC-Dose to Medium (MC-Dm) ve MC-Dose to Water (MC-Dw) hesaplamalarının CC04
ve EBT-XD ölçümleri ile uyumlu olduğu sonuçlarda maksimum %5 fark bulundu. Sonuçlar doz
hesaplamaları açısından değerlendirildiğinde MC-Dw modunun kemik içerikli inhomojen
fantomda daha yüksek doz hesapladığı görüldü. Özellikle 1x1 cm2 alanda CC algoritmasında %8,
PB algoritmasında %14 e varan doz farklılıkları oluştuğu gözlendi. Yinede 2x2 cm2 den daha
büyük alanlarda inhomojen fantomda maksimum %5 civarı farklar bulundu.

Monte Carlo (MC), Collapse Cone (CC), Pencil Beam (PB) algorithms are used to
calculate dose distribution in radiotherapy. The accuracy of the algorithms and small field
measurements are critical for accurate calculation of dose distribution in inhomogeneous
environments. In this study, in order to investigate the dosimetric differences of dose calculation
algorithms in inhomogeneous environments, measurements were taken in inhomogeneous
phantom with CC04 (IBA, Germany) ion chamber and EBT-XD (Ashland, USA) film and
compared with the treatment planning system (TPS). Computed tomography (CT) images taken
with CC04 ion chamber and EBT-XD film were transferred to TPS, and calculations were made
according to 1x1, 2x2, 3x3, 4x4 and 5x5 cm2 field and 6MV-FFF, 10MV-FFF energies. Then, the
same phantom setup was set up on ElektaVersa HD linear accelerator and measurements were
taken. A maximum difference of 5% was found in the results where the calculations of MC-Dose
to Medium (MC-Dm) and MC-Dose to Water (MC-Dw) were consistent with the CC04 and EBTXD
measurements. When the results were evaluated in terms of dose calculations, it was seen that
the MC-Dw mode calculated a higher dose in the bone-containing inhomogeneous phantom. It
was observed that dose differences of up to 8% in the CC algorithm and up to 14% in the PB
algorithm, especially in 1x1 cm2 area. However, in areas larger than 2x2 cm2, a maximum of 5%
differences were found in the inhomogeneous phantom.
