Mikrovasküler hastaliğin miyokard perfüzyon sintigrafisi ve speckle tracking ekokardiyografi ile değerlendirilmesi

Özet:

 The efficacy of 2D speckle tracking echocardiography in the diagnosis of microvascular disease and its comparison with myocardial perfusion scintigraphy (MPS) in the detection of ischemia in this patient group. Materials and Methods: Forty stable angina pectoris patients (25 females, 58.85 ± 9.73 years) who had ischemia in MPS and had non-obstructive CAD (<50% stenosis) on coronary angiography were included in the study. Strain analysis was performed by 2D speckle tracking echocardiography in all patients. Longitudinal strain was measured for 17 LV myocardial segments. LV segments were grouped and compared as ischemic and non-ischemic segments according to MPS results. Results: The mean longitudinal strain values of the segments with ischemia were -20.24 ± 6.00% and of the segments without ischemia were -20.11 ± 6.46% and no statistically significant difference was observed (p> 0.05). Subgroup analysis of sex, hypertension, diabetes mellitus, hyperlipidemia and hypertriglyceridemia showed no difference in longitudinal strain between the ischemic and non-ischemic segments. Conclusions: Left ventricular 2D longitudinal strain analysis with speckle tracking was not sufficient for the diagnosis of microvascular disease. Again in this patient group, STE cannot detect ischemia detected by MPS. This technique should also be evaluated in larger studies.
