AMAÇ: Bu çalışmada, travmaya bağlı olarak 11 nolu dişinde avülsiyon, 12, 21, 22 nolu dişlerinde lüksasyon teşhisi konulmuş ve üst çene bukkal kortikal kemik kırığının eşlik ettiği bir olguda uygulanan endodontik tedavi yaklaşımının sunulması amaçlanmıştır. OLGU SUNUMU: Sistemik olarak sağlıklı kadın hasta kliniğimize travmaya bağlı üst kesici dişlerinde meydana gelen yaralanma şikâyeti ile başvurmuştur. Hasta anamnezi sonrasında travmanın bir ay önce gerçekleştiği anlaşılmıştır. Klinik ve radyografik muayenede; 11 nolu dişin avülse olduğu ve 12, 21, 22 nolu dişlerin ise lükse olduğu, ayrıca maksilla bukkal kemikte fraktür olduğu anlaşılmıştır. Vitalite testleri tüm dişlerin devital olduğu ortaya çıkarmıştır. İlgili dişlerde perküsyon ve palpasyon hassasiyet tespit edilmiştir. Ortodontik tel ve kompozit rezin kullanılarak 13, 23 nolu dişler arasına splint uygulanmıştır. Akabinde tüm dişlere endodontik tedavi başlanmıştır. Haftada bir kez yenilenen Ca(OH)2 medikasyonunu takiben 4. haftada kök kanal tedavisi tamamlanmıştır ve aynı seansta splint uzaklaştırılmıştır. Hastanın 1. 3. ve 6. aydaki takip randevularında dişler asemptomatik ve fonksiyondaydı. Hastanın takibi devam etmektedir. SONUÇ: Bu olgu sunumu, bukkal kemik fraktürü ve birden fazla lüksasyona uğramış dişleri içeren vakaların travmadan sonra 1 ay kadar süre geçmiş olsa dahi endodontik yaklaşım ile sağlıklı bir şekilde ağızda tutulabileceği göstermektedir.

INTRODUCTION: In this study, the aim was to present the endodontic treatment approach applied in a case
report with avulsion in tooth #11 and luxation in teeth #12, #21, and #22, accompanied by a buccal cortical bone
fracture in the maxilla, which occurred due to trauma.
CASE REPORT: A systemically healthy female patient presented to our clinic with complaints of injury to
her maxillar incisor teeth due to trauma. After obtaining the patient’s medical history, it was determined that
the trauma occurred one month ago. During the clinical and radiographic examination, it was determined
that tooth #11 was avulsed, and teeth #12t, #21, and #22 were luxated. Additionally, a buccal bone
fracture in the maxilla was identified. The vitality tests, revealed that all teeth were non-vital. Percussion
and palpation tenderness were detected in the affected teeth. An orthodontic wire and composite resin
were used to apply a splint between teeth #13 and #23. Subsequently, endodontic treatment was initiated
for all teeth. After the weekly renewal of Ca(OH)2 medication, root canal treatment was completed in the
fourth week, and the splint was removed during the same session. The patient’s teeth were asymptomatic
and functional during the follow-up appointments at 1, 3, and 6 months. The patient’s follow-up is ongoing.
CONCLUSION: This case report presentation demonstrates that even in cases involving buccal bone fracture and
multiple luxated teeth, a healthy retention in the oral cavity can be achieved through an endodontic approach,
even when a considerable amount of time has passed since the trauma, up to one month.
